How Much Do No-Shows Cost Your Dental Practice? A Simple Way to Work It Out

Every clinic has them: the patient who booked on Sunday, confirmed nothing, and simply did not turn up on Tuesday. One empty chair rarely feels like a crisis. But a few missed appointments every week quietly become one of the largest losses in a dental practice, and most clinics never put a number on it.
This guide shows you how to calculate that number in two minutes, what part of it you can realistically win back, and where to start. If you prefer to skip the arithmetic, the free No-show Cost Calculator does it for you.
The formula
You only need three numbers:
- No-shows per week: appointments where the patient did not attend and did not cancel in time to rebook the slot.
- Average visit value: what a patient typically pays for one visit, not the value of a whole treatment plan.
- Weeks per month: use 4.3, the average number of weeks in a month.
Monthly cost of no-shows = no-shows per week × 4.3 × average visit value. Multiply by 12 for the yearly figure.
A worked example
Take a clinic with 6 no-shows a week and an average visit value of 800 EGP:
- Missed appointments per month: 6 × 4.3 = 25.8, so about 26
- Monthly value of missed appointments: 25.8 × 800 = 20,640 EGP
- Yearly value: 20,640 × 12 ≈ 247,700 EGP
These are illustrative numbers, not market averages. Use your own appointment book. And remember the real cost is higher than this: the team, the rent and the equipment are paid whether the chair is full or not, and a patient who misses a visit often drops out of treatment altogether.
How much can reminders recover?
Not every no-show is preventable. Some patients get ill, some forget, and some never intended to come. But a large share of missed appointments happen for simple reasons: the booking was made days earlier, nobody reminded the patient, or there was no easy way to cancel and rebook.
A structured reminder system fixes most of those reasons. Be conservative when you estimate the effect. If you assume reminders recover 40% of missed visits in the example above, that is about 8,250 EGP a month back in the clinic, for a process that costs almost nothing to run.
A reminder system that works
- Confirm at booking. Send the date, time, address and a map link on WhatsApp straight away.
- Remind the day before. Ask the patient to reply “Confirm” or “Change”. A reply you can act on is worth more than a reminder they ignore.
- Remind two hours before for longer or high-value appointments.
- Make rescheduling easy. A patient who can move an appointment with one message is less likely to just disappear.
- Follow up every no-show the same day with a friendly message and an offer to rebook. Silence tells the patient nobody noticed.
- Keep a short waiting list so you can fill late cancellations.
You do not need to write these messages from scratch. The WhatsApp Reply Generator creates confirmation, reminder and no-show follow-up messages with your clinic details, ready to save as WhatsApp Business quick replies.
Measure it every week
Count no-shows weekly and write the number down. It is the only way to know whether the reminders are working, and it shows you patterns: specific days, specific treatments, or bookings made too far in advance. Frequently asked questions
What counts as a no-show?
Any appointment where the patient did not attend and did not cancel early enough for you to offer the slot to someone else. Late cancellations that leave the chair empty belong in the same count.
Should I charge a no-show fee?
Some clinics take a small deposit for long appointments. It can work, but start with confirmations and reminders first: they solve most of the problem without making patients feel penalised.
Does the visit value include the whole treatment plan?
No. Use the value of a typical single visit so the estimate stays realistic. The lost treatment plans are an extra cost on top of this figure.
